Field Trip Reservation Request Form

NJMSC/NJSG field trips can be requested on-line. Before completing this request form, please review our Field Trip Programs to select the best one for your group. You will need to provide a first, second and third choice date.

Your request will be acknowledged by email and confirmed by regular mail. Submitting this form does not constitute a reservation.

Read this form to verify that all information is correct. If you have questions or changes, contact the NJMSC/NJSG Education Department immediately.
A non-refundable $50.00 minimum deposit is due within 30 days of Reservation Confirmation.
